Medical & Supplements add-on — terms for coaches
Effective date: 21 October 2026
What Phorm does. Phorm gives you a place to keep a record of the medication, supplements and similar items your client uses. Phorm does not prescribe, recommend, sell, supply or administer any medicine, supplement or other substance, and nobody at Phorm reviews what you record.
What you’re responsible for. You agree that you will:
- record only what your client lawfully uses, or has been prescribed by a registered healthcare practitioner;
- stay within your qualifications, and refer medical questions to a doctor, pharmacist or other registered practitioner;
- comply with the laws that apply to you, including the Medicines and Related Substances Act and POPIA;
- get your client’s consent before recording anything. Phorm asks them for it, and you can’t record until they agree.
What you must not do. You must not use Phorm to plan, encourage, supply or administer any substance unlawfully. That includes scheduled substances without a valid prescription.
Liability. To the extent the law allows, C-Suite Holdings (Phorm) is not liable for any loss, injury or harm that arises from the use of any substance, from unauthorised or unlawful use of this add-on, or from reliance on what is recorded in it. You indemnify C-Suite Holdings against claims arising from unauthorised or unlawful use of the add-on by you or your team.
Misuse. If we have reason to believe the add-on is being misused, we may switch it off for your business and keep the records needed to deal with it.
